Vibhaṅgasutta

[14] And what, bhikkhus, is name-and-form (nāmarūpa)? Feeling (vedanā), perception (saññā), volition (cetanā), contact (phassa), and attention (manasikāra) — this is called name. The four great elements (mahābhūta) and the form derived from the four great elements — this is called form. Thus this name and this form — this, bhikkhus, is called name-and-form.
